Pharmacokinetics
Gliclazide belongs to the group of intermediate acting hypoglycaemic agents. It is rapidly absorbed from the GIT, and is metabolised in the liver. It appears in the blood within 1-2 hrs. It is extensively plasma protein bound ( more than 90%). Peak level is achieved in 4-6 hrs. The plasma t 1/2 is 8-12 hrs and its duration of action is 12 hrs.
Indications
Gliclazide belongs to second generation sulfonylureas. It is indicated in: Non insulin dependant diabetes milletus, maturity onset diabetes and in insulin resistant diabetes.It stimulates insulin secretion by beta cells of the pancreas. Gliclazide can inhibit platelet aggregation and increase fibrinolysis,
Routes of Administration and Dosage
For Noninsulin dependant diabetes, oral dose: The drug is increased in stepwise fashion. It is started as 40-80 mg/day in divided doses given 1/2 hr before breakfast and evening meal. It can gradually be increased to 320 mg/day which is also given in div
Contra Indications
Gliclazide is contraindicated in following conditions: pregnancy, lactation, hypersensitivity, renal impairment, juvenile onset diabetes, unstable or brittle diabetes, diabetic ketoacidosis .
Precautions
Precaution is to be taken in hypoglycaemia and stress. During metabolic stress insulin may be needed in addition. Extreme care should be taken during transfer of drug therapy.
Interactions
The hypoglycaemic effect of gliclazide is potentiated by Phenylbutazone, aspirin, clofibrate, sulphonamides, oral anticoagulants and MAO inhibitors. Rifampicin, barbiturates, alcohol, diuretics, diazoxide, glucocorticoids,estrogens and sympathomimietic drugs antagonise the hypoglycaemic action of gliclazide.
