Pharmacokinetics
When given orally it undergoes extensive first pass metabolism hence it is administered sublingualy.
Indications
Nitrates are used to treat the symptoms of angina . Nitrates are used to treat angina in three ways: to relieve an attack that is occurring by using the drug when the attack begins; to prevent attacks from occurring by using the drug just before an attack is expected to occur; or to reduce the number of attacks that occur by using the drugs regularly on a long-term basis.
Routes of Administration and Dosage
For angina : For chewable dosage form (tablets): Adults: 5 mg every two to three hours, chewed well and held in mouth for one or two minutes. For buccal or sublingual dosage form (tablets): Adults: 2.5 to 5 mg every two to three hours.
Contra Indications
Contraindicated in some cases of Acute MI, Severe anaemia, cerebral haemorrhage, hypotension, obstructive cardiomyopathy, raised intra cranial tension and pregnancy.
Precautions
Headache is a common adverse effect. Dizziness or lightheadedness may be more likely to occur in the elderly. Anemia (severe) and Glaucoma: May be worsened by nitrates.In Intracerebral hemorrhage (Recent): Nitrates may worsen the condition. In recent Myocardial Infarction : Nitrates may cause hypotension, which can aggravate problems associated with MI. In renal disease and hepatic disease : Effects may be increased because of slower removal of nitroglycerin from the body.
Interactions
Orthostatic hypotension occurs when used together with: calcium channel blockers, phenothiazines or tricyclic antidepressants.
