Category Antimalarial Agent- Rapid acting erythrocytic schizonticide.


Pharmacokinetics

Mefloquine is given as a single oral dose as it has a long plasma half life of 2 weeks. The absorption of the drug is enhanced in the presence of food. It is highly bound to the protein and is concentrated in the RBC. It is metabolised in the liver and is mainly excreted in the faeces.

Indications

Mefloquine belongs to the 4-quinoline methanol group. It acts as schizonticide against P.falciparum and P.vivax.Mefloquine is used for prevention and treatment of chloroquine-resistant and multidrug-resistant plasmodium-falciparum.

Routes of Administration and Dosage

Oral dose -ForTreatment of chloroquine resistant and multi resistant falciparum malaria :15 mgs per kilogram [1-1.5 gram total] single dose.For supressive prophlaxis: 250 mg weekly or 500 mg fortnightly has been sucessfully used in areas where chloroquine resistant falciparum is prevalent.

Contra Indications

The drug is contraindicated in those with hypersensitivity to quinine & related compounds and in children under 15 kgs weight.

Precautions

Mefloquine may make Convulsions, Epilepsy, Heart block and Psychiatric disorders worse.

Interactions

Use of Diltiazem, Flunarizine, Isradipine, Nicardipine,Nifedipine, Nimodipine, Quinidine, Quinine, Verapamil: together with mefloquine may result in bradycardia and other cardiac disorders; also, an increased chance of convulsions may occur when quinine is taken together with mefloquine. Use of Chloroquine with mefloquine may increase the chance of convulsions. Use of Divalproex,Valproic acid together with mefloquine may result in low blood levels of Valproic acid and an increased chance of convulsions. Use of oral Typhoid vaccine with mefloquine may reduce the efficacy of the vaccine.
